Description

Sclerotherapy is a treatment for lymphatic system and blood vessel malformations. The vessels become smaller after a medication is injected into them. Children and young adults with vascular or lymphatic malformations benefit from it. Sclerotherapy is one method (along with surgery, radiofrequency, and laser ablation) for the treatment of spider veins, occasionally varicose veins, and venous malformations in adults. Sclerotherapy is also used to treat hemorrhoids, hydroceles, and smaller varicose veins. In ultrasound-guided sclerotherapy, the underlying vein is seen with an ultrasound so the doctor can administer and monitor the injection. After venous abnormalities have been detected by duplex ultrasound, sclerotherapy is frequently performed under ultrasound guidance. Sclerotherapy with microfoam sclerosants and ultrasound guidance has been demonstrated. Control reflux from the sapheno-popliteal and sapheno-femoral junctions effectively. This is because of the development of additional compelling innovations, including laser removal and radiofrequency, which have exhibited better viability than sclerotherapy for treatment of these veins.

When a sclerosing solution is injected into the unwanted veins, the target vein immediately shrinks and then dissolves over several weeks as the body naturally absorbs the treated vein. Sclerotherapy is a non-surgical treatment that only takes about ten minutes to complete. Sclerotherapy is the "gold standard" and is preferred over laser for eliminating large spider veins (telangiectasiae) and smaller varicose leg veins. Telangiectasia is conditions in which broken or widened small blood vessels that sit near the surface of the skin or mucous membranes create visible. In contrast to a laser, the sclerosing solution also closes the "feeder veins" under the skin that are causing the spider vein. Different infusions of weaken sclerosant are infused into the unusual surface veins of the elaborate leg. After that, stockings or bandages are used to compress the patient's leg, which they usually wear for a week after treatment. During that time, patients are also encouraged to walk frequently. To significantly improve the appearance of their leg veins, the patient typically needs at least two treatment sessions spaced several weeks apart.

Microfoam sclerosants under ultrasound guidance can also be used in sclerotherapy to treat larger varicose veins, such as the great and small saphenous veins. Following the creation of an ultrasound map of the patient's varicose veins, these veins are injected while real-time ultrasound monitoring of the injections is carried out. The sclerosant can be noticed entering the vein and further infusions performed with the goal that every one of the unusual veins are dealt with. The treatment of treated veins is confirmed by follow-up ultrasound scans and any remaining varicose veins can be identified and treated.

Preclinical studies also indicated that electroporation in conjunction with bleomycin impaired the barrier function of the endothelium by interacting with the organization of the cytoskeleton and the integrity of the junctions. Bleomycin electrosclerotherapy consists of locally delivering the sclerosant bleomycin and applying short high voltage electrical pulses to the area to be treated. This causes a local and temporary increase in permeability of the cell membranes, increasing the intracellular concentration. A retrospective study of 17 patients with venous malformations who did not respond to previous invasive therapies showed an average decrease in lesion volume measured on MRI images of 86% with clinical improvement in all patients after an average of 3.7 months and 1.12 sessions per patient, with a reduced dose of bleomycin and a reduced number of sessions compared to standard bleomycin sclerotherapy.

Sodium Tetradecyl Sulfate

The evidence supports the current place of sclerotherapy in modern clinical practice, which is usually limited to treatment of recurrent varicose veins following surgery and thread veins, according to a cochrane collaboration review of the medical literature. A second cochrane collaboration review comparing sclerotherapy to surgery came to the conclusion that sclerotherapy has greater short-term benefits than surgery, but surgery has greater long-term benefits. "At one year, sclerotherapy was superior to surgery in terms of treatment success, complication rate, and cost; however, after five years, surgery was superior. A health technology assessment found that sclerotherapy provided less benefit than surgery, but is likely to provide a small benefit in varicose veins without reflux from the sapheno-femoral or sapheno-popliteal junctions. However, the evidence was of poor quality, and additional research is required. It did not investigate the relative advantages of surgery and sclerotherapy for junctional reflux varicose veins. Although skin necrosis is uncommon, it can be cosmetically "potentially devastating" and may take months to heal if the sclerosant is injected properly into the vein. However, if it is injected outside of the vein, tissue necrosis and scarring may occur. When diluted Sodium Tetradecyl Sulfate (STS) concentrations of less than 0.25 percent are used, it is extremely uncommon, but when higher concentrations (3%) are used, it has been observed. When STS is injected into arterioles-small artery branches-the skin frequently becomes white. The majority of complications result from an intense inflammatory reaction to the sclerotherapy agent in the area surrounding the injected vein. There are also systemic issues that are now becoming more and more understood. These take place as the sclerosant travels through veins to the brain, heart, and lungs. A stroke was blamed in a recent report on foam treatment, which involved injecting an unusually large amount of foam. Recent studies have demonstrated that even a trace amount of sclerosant foam injected into the veins causes rapid bubble formation in the brain, heart, and lungs. The introduction of duplex ultrasonography in the 1980s and its subsequent incorporation into sclerotherapy were the next significant advancements in the field of sclerotherapy. Deep vein thrombosis carries a risk of pulmonary embolism (a very rare complication of sclerotherapy), an emergency situation where the clot travels from your leg to your lungs and blocks a vital artery. Seek immediate medical care if you experience difficulty breathing, chest pain or dizziness, or you cough up blood. Tiny air bubbles may rise in your bloodstream. These don't always cause symptoms, but if they do, symptoms include visual disturbances, headaches, fainting and nausea. These symptoms generally go away, but call your doctor if you experience problems with limb movement or sensation after the procedure.
